CYS School Liaison Office for In and Out Processing purposes  is located on the first floor of Building 110 (northeast corner). Appointments can be made by calling (309) 782-6515. Hours of operation are 8 a.m. to 4:30 p.m.

When entering child(ren) in new school remember to bring...

· Sponsors Orders

· Birth Certificate

· Immunization Records

· Passport, ID  Card and Social  Security Card

· Transcript/School Records/Last Year’s Report  Card

· Sports physical evaluation record, signed by a physician (valid for one year. Sports physical examination may be coordinated with the medical clinic.

· Money for school/sports fees.

· Copy of school records

· Course descriptions

· All documents of achievements and awards

Rock Island Arsenal CYS In and Out Processing Procedures

Per AR 60-8-101, all soldiers who have children 0 – 18 years of age must in and out process through Child and Youth Services/School Liaison Services (CYS/SLS).  The CYS/SLS office is located in the Building 150, Rodman Avenue, (309) 782-6515.

In-processing

(1)   The respective Military Personnel Office of any military tenant of the U.S. Army-Rock Island Arsenal will inform soldiers of the requirement to in-process through Child and Youth Services/School Liaison Services (CYS) located in Building 110, Rodman Avenue.  All soldiers with children ages 0 to 18 years are required to in and out process through CYS/SLS.

(2)   The MPO will provide their respective In-processing Personnel Record, to each soldier. 

(3)   CYS/SLS staff will provide soldiers information concerning local schools, child care options, and youth programs.  Staff will sign soldier’s in-processing papers. 

Out-processing 

(1)   The respective Military Personnel Office of any military tenant of the U.S. Army-Rock Island Arsenal will notify Soldiers of the requirement to Out-process through CYS/SLS at the time of their levy briefing or, if an Officer or NCO, when they submit their Request for Orders (RFO).  The soldier has no more than 10 days to clear all required agencies on the installation. 

(2)   The Military Personnel office of the respective tenant will provide an Installation Clearance Record Form to the soldiers. 

(3)   Soldiers will be instructed to take the Student Withdrawal form, which can be picked up at Child and Youth Services, to the public school which their child last attended. School personnel will review student’s records to ensure that all fees have been paid and that any outstanding material has been returned.  School personnel will sign the withdrawal form and return form to Soldier.  

(4)   Soldier will take the signed withdrawal form to the CYS office located in building 110, Rodman Avenue.  A CYS/SLS staff member will sign and date the soldier’s Installation Clearance Record and provides information and transitional assistance if needed.  Parents of home schooled children must provide proof of their home school status in order for CYS/SLS staff to sign clearance record.  Soldiers clear all CYS programs at this location.
