Waitlist Form
There is sometimes a waitlist for childcare services. Due to this factor, it is important to place your child on the waitlist as soon as possible if you are contemplating using our childcare services. Please call the CYSS Office at (309)782-3844 to request a waitlist/application packet. The waitlist form can also be accessed below on this webpage. Please feel free to also call this number with any questions you may have. We look forward to the opportunity to work with you and your family!
